Physician Benefits
Overview
Medical Plans
Premium for provider paid by CMP. Family coverage option available
through payroll deduction. Coverage begins the first of the month
following date of hire. Choice of two medical plans, including an HMO and
PPO option. No annual deductibles. Office visit and generic prescription
co-pays. Additional compensation available for those waiving coverage.
Dental Plan
Premium for provider paid by CMP. Family coverage option available
through payroll deduction. Coverage begins the first of the month
following date of hire. CMP offers a PPO Dental Plan with 100% coverage
for preventative services, 80% for basic services. $50 annual deductible.
Vision Plan
Premium for provider paid by CMP. Family coverage option available
through payroll deduction. Coverage begins the first of the month
following date of hire. The vision plan includes an eye examination,
lenses and frames once every 12 months.
Professional Liability
Coverage
Premium paid by CMP. Coverage begins on
date of hire. Offers protection against the costs associated with
professional liability claims, which arise from medical care to patients
provided by you within the course and scope of your employment with
CMP.
401k Retirement Plan
Participation available to all employees on date of hire. Employer match
up to 3% of base salary. Five year vesting schedule.
Long Term Disability
Insurance
Premium paid by CMP. Provides up to 60% or $6,000 of monthly base salary.
Life
Insurance
Premium paid by CMP. Term life insurance benefit of $100,000 coverage.
Short Term State
Disability Insurance
California State Disability Insurance is a partial wage replacement
insurance plan for non-work related illness or injury.
Continuing Medical
Education (CME)
One week of time off provided per year. $2000 annual reimbursement program
for qualifying expenses, which includes CME expenses, professional society
dues, and license fees.
Paid Time Off
Annual accrual equivalent to three weeks vacation/sick, plus eight
holidays. Accrues each pay period beginning on date of hire.
Annual Incentive
Program
Based upon productivity and patient satisfaction results.
Shareholder Program
Shares are issued annually to our providers
and the number of shares issued is based on annual salary.
Employee Assistance
Program
A confidential counseling program designed
to help employees and their families through difficult and stressful
times. Three free visits per six-month period and referrals, if needed.
