
Routine Check-ups
These visits are required at the Care Center Pediatric office (check with your provider if you visit another CMP site.) For more information on each vaccine, click on the name of the vaccine or check out our Immunization Reaction Sheet.
Well Child Visits*:
| 1 – 2 weeks Feeding Evaluation | |
| 2 months Immunizations: DTaP, HepatitisB/Hib (Comvax), polio (IPV) [3 shots] | |

*Babies with problems need more frequent visits
Immunization Information
| Hepatitis B – Every child now needs a series of these 3 important shots. Older children are required to receive this series if they have not received them as infants. | |
| Comvax – A combination of the Hepatitis B and Hib vaccines that eliminates one extra shot at the 2, 4 and 12 month visit and eliminates the need for a Hib shot at 15 months. | |
| Hepatitis A – Given by request only | |
| Polio (IPV) – Inject able killed vaccine is now given at 2 mos., 4 mos., 12 mos. and 4 years. Oral live vaccine is no longer necessary. | |
| Chicken Pox – Only one shot, given at 12 months or older. If not given until 13 years of age the vaccine is 2 shots, spaced one month apart. | |
| Flu Vaccine – Recommended for children with asthma or other chronic diseases. It can be given in October-November on a yearly basis. | |
| Fluoride/Vitamin Drops – Given at 6 months as a liquid drop and continued throughout the first two years. Chewable tablets will be prescribed after two years of age. If taken regularly, fluoride can reduce cavities by 60% or more. Fluoride supplement is not needed if your water is fluoridated, call the city water department to check your area. |
