Advance Directives

Taking care of yourself and your family can include making decisions about medical care while your health allows you to do so.

Although death comes to everyone, many of us tend to fear its approach and may avoid confronting the issues surrounding the end of life.  Nevertheless, the providers of CMP feel it is important for each person to document his or her wishes prior to serious illness or physical or mental disability.  Otherwise, those wishes may not be known and may not be honored.  Not having these desires known may create unnecessary stress on your loved ones at an already difficult time in their life.

CMP is pleased to present information for you on this subject to help make clear your rights, options and opportunities. If you already understand the situation and are simply interested in getting forms to fill out, go here.  Please contact your CMP office for any questions not answered on this site.

This section made possible through an educational grant from Aventis pharmaceuticals, makers of Allegra.
