Pick up the form from your lawyer, hospital, or state attorney general’s office. You do not need a lawyer for an AD to be legally valid. You can also pick up a free form from the California Coalition for Compassionate Care web site. Go here for more information for obtaining forms

Your CMP provider recommends that you use the forms provided by the California Coalition for Compassionate Care, a statewide collaboration of over 50 organizations representing healthcare providers, consumers, and state agencies committed to improving end-of-life care for Californians.
